| | Case Manager, Supportive Living Program
Responsibilities:- Assist eligible clients with application process for housing, by actively assisting them with Supportive Living Program application, paperwork procedures and program entry.
- Establish and maintain supportive relationship with families participating in Supportive Living Program.
- Monitor each client's active participation in the program.
- Monitor client compliance with program guidelines through home visits on consistent schedule.
- Assist women in identifying problems, setting goals and making decisions.
- Provide advocacy, group and/or individual counseling for clients in the program.
- Provide options to violent behaviors to women through role modeling and education.
- Provide input and feedback about Life Skills Program. The program includes budgeting, financial planning, health and nutrition, self-improvement and socialization activities.
- Provide consultation, education and assistance to women in the program.
- Work with Operations Manager to provide for client's physical needs (furniture, etc.).
- Provide training, supervision and support to volunteers and/or interns in the program.
- Participate in weekly case conference and individual supervision with Program Director
- Maintain accurate and current client records. Maintain requested statistical data, and other documents as required. Keep records as needed to provide reorts on program status when requested by HUD, other grants or supervisor.
- Represent agency in special groups or coalitions and community education activities.
- Participate in in-service training program and workshops as indicated.
- Coordinate program with bilingual counselor and primary case manager to provide services for Spanish speaking clients.
- Maintain working schedule as required by program with adequate flexibility.
- Maintain current professional knowledge of domestic violence and crisis theory, including assessment of danger, lethality and client resources.
- Perform all related duties as assigned by the Executive Program Director.
Requirements:- Master's Degree in Social Work, Psychology, Counseling or related Human Services field.
- One to two years experience in crisis intervention and/or domestic violence issues preferred.
- Counseling and group experience required.
