Case Manager, Homelessness Prevention and Rapid Re-housing Program (HPRP)

Part-Time (20 hours per week)/Exempt Reports To:  Program Director

Provide case management and housing related assistance to client families.  Assist in identifying client needs, offer appropriate information regarding resources, and assist in developing a service delivery plan.  Act as an advocate for clients in obtaining community services and establishes referral relationships with other agencies to facilitate services.

Other duties include but not limited to:

  • Provide case management services which includes, but not limited to housing, employment and legal and medical referrals;
  • Assist the client in developing a safety plan;
  • Assess hotline calls using appropriate crisis intervention techniques and skills obtained through training; make appropriate referrals as needed;
  • Respond appropriately to emergency situations;
  • Act as a liaison with community agencies to promote appropriate service delivery to client families;
  • Establish referral relationship with other agencies to facilitate services;
  • Act as a legal advocate for clients;
  • Provide counseling services to clients, including individual, group and family;
  • Work assigned schedule, alternative hours may be required;
  • Maintain current professional knowledge of domestic violence and crises theory, including assessment of danger and resources of direct solution;
  • Maintain accurate and current client records; assist in providing all reports and/or statistical information required by the Program Director;
  • Attend required case conference/staff meetings;
  • Participate in in-services, training programs, community education activities and any other continuing education sessions;
  • Perform other duties as assigned by the Program Director or Executive Program Director.

      Requirements:

      Bachelors Degree or equivalent experience in Social Work, Psychology or related Human Services field; three+ years experience in crisis intervention and/or domestic violence preferred.
